Students attending St. Joseph
Catholic School must have the proper health forms completed
before enrollment. These forms are:

New Student Physical - All
new students are required to have a comprehensive physical
examination. This must
be signed by a licensed physician. Please click on

Health Report Form
• Immunization Form - All students must have a current immunization
form, and present it to the school before a child begins
the school year. Please contact your local health department
or your family physician in order to secure the form.
• Ear, Eyes and Dental - Students must have a completed
form on file with the school prior to the beginning
of the school year. Please contact your local county health
department or your family physician in order to secure
the form.
